YOUR PRIVACY - INFORMATION GATHERING
On occasion, in addition to the information you have
already provided for us, we will need additional information or will want to
verify particular information. A NOTICE OF INSURANCE INFORMATION PRACTICES will
be provided to assist you in understanding how we gather information and our
compliance with privacy laws.
In addition, Federal law (Fair Credit Reporting Act)
requires that we provide certain procedural information. As a part of our
normal underwriting procedure for processing applications, renewals and
reinstatements, an investigative consumer report may be made, as to your
insurability. This inquiry includes information obtained through personal
associates, financial sources, friends, neighbors, or others with whom you are
acquainted and typically includes information as to your character, general
reputation, personal characteristics and mode of living. The information
gathered may in certain circumstances be disclosed to third parties without
authorization. You have the right to make a written request within a reasonable
period of time for a complete and accurate disclosure of additional information
concerning the nature and scope of the investigation.
DISHONORED PAYMENT OF INITIAL PREMIUM
If your initial premium payment is dishonored by the
bank on which payment is drawn, NO contract of insurance has existed.
NOTE: THE INSURER MAY ELECT TO CANCEL COVERAGE AT ANY TIME DURING THE
FIRST 59 DAYS FOLLOWING ISSUANCE OF THE COVERAGE FOR ANY REASON WHICH IS NOT
SPECIFICALLY PROHIBITED BY STATUTE.

In order to evaluate your application for insurance,
we occasionally need certain additional information. Most of the information
you have already provided us. The purpose of this notice is to inform you of
your privacy rights concerning information we gather from other sources and our
compliance with privacy laws
What Type Of Information Is
Collected About You
On occasion, we will need additional information or
will want to verify particular information.
Therefore, we gather additional information from other sources, such as
motor vehicle records, consumer reports and claim reports.
Motor Vehicle Records
When you apply for, renew,
change or reinstate your auto insurance, we order a copy of your motor vehicle
record from your state’s motor vehicle department. Your motor vehicle record
contains information about accidents and violations of state traffic laws.
Consumer Reports
When you apply for, renew,
change or reinstate your auto insurance, we may order a consumer report from an
outside consumer reporting agency. The consumer report contains information
about credit history. If we take adverse action on your policy because of
information contained in your consumer report, we will provide a written notice
to you with the name and address of the consumer reporting agency. You have the
right to request a copy of your consumer report and correct any information
contained in the report by contacting the consumer reporting agency.
Claim Reports
When you apply for, renew,
change or reinstate your auto insurance, we may order a claim report from an
outside insurance-support organization. This organization maintains a database
which includes information about insurance claims you have had in the past. We
will also submit information about your claims with us to this organization.
Disclosure Of
Personal Information
We usually do not disclose information about you to
others without your consent. However, in some circumstances we may disclose
information to others without your prior authorization. The most common
disclosures are to the following persons:

Other
insurance companies, agents, or consumer reporting agencies in connection with
any application, policy or claim involving you.

Persons
who need information to perform a professional, business or insurance function
for us, provided they agree not to disclose the information further without
your written authorization.
·
Persons
or organizations, including law enforcement agencies, who need information to
determine eligibility for insurance benefits or payments; to detect or prevent
criminal activity, fraud, misrepresentations; or material nondisclosure in
connection with an insurance transaction.

Medical
professionals to verify your insurance coverage or to verify the individuals
treated by the medical professional.
·
Governmental
authorities to determine your eligibility for government health benefits.
·
Certificate
holders or policyholders to provide information about the status of an
insurance transaction.
·
Lien holders,
mortgages, assignees, lesser, or other persons having a legal or beneficial
interest in your policy, provided the information disclosed is limited to that
which is reasonably necessary to permit such person to protect its interests in
the policy.
·
Adjusters,
appraisers, or investigators for the purpose of investigating or settling
claims.
· Authorized personnel of the Division of Motor Vehicles upon request.
