Section 1: Important Notice About the Protection of Your Privacy
At Delos Insurance Company (“Delos”) and our program administrator Educators Personal Insurance
Center Agency, Inc. (“EPIC”) we value your business and the trust you’ve placed in us. That’s
why we want to assure you that we are serious about keeping your personal information private
before, during and after your business relationship with us. We hope that this Notice will help
you understand how we treat customer information we obtain from you or other sources in the course
of providing you with products and services. We want you to know that Delos respects your privacy
and protects the confidentiality of your information. It is important to us that you know that:
Section 2: Information We Collect.
During the course of providing products and services we collect non-public personal
information about you. Much of the personal information that we collect comes directly
from you. When submitting your application or request for insurance or other products and
services we offer, or in requesting an insurance quote, you may give us information such as
your name, address and Social Security number. We also keep information concerning your
transactions with our affiliates, others, or us, for example, the types of products and services
that you purchase from us, premiums, account balances and payment history.
Section 3: Information We May Disclose to Third Parties.
Delos does not sell your personal information to anyone. Nor do we normally share it
with companies or organizations outside of Delos. However, in some circumstances we may disclose
information to others without your prior authorization. For example, in order to procure other
components of the insurance products you request and service those insurance products we will
need to provide your information to certain parties. If you prefer we not share any information
with parties not affiliated with Delos or EPIC please notify us. Note, this may affect our
ability to provide insurance services and products to you.
Section 4: Our Practices Regarding Information Confidentiality and Security.
We restrict access to nonpublic personal information about you to those employees who need to
know that information to provide products or services to you. We maintain physical, electronic,
and procedural safeguards that comply with federal regulations to guard your nonpublic personal
information.
Section 5: How You Can Find Our What Information We Have About You
You have a right upon written request to us, to obtain a copy of any recorded personal information
we have in our files. Your request must contain your full name, address, and policy number.
We will, within 30 business days from the date we receive your request, send you a copy of the
information we have. We will also tell you the identity, if known, of those persons to whom we
have disclosed personal information within the prior two years. If the identities of these
persons are not known, we will tell you the names of those persons to whom we normally disclose
such information. There are some types of information, however, to which we are not required to
give to you access, such as information collected when we evaluate a claim or when the possibility
of a lawsuit exists.
We may also obtain information from outside sources, including:
This information may include loss information reports, motor vehicle reports, credit reports
and medical information.
We may, without authorization but only as permitted or required by law, provide personal
information to persons or organizations both inside and outside of Delos in order to fulfill a
transaction you have requested, service your policy, market our products to you, investigate and/or
handle claims, detect and/or prevent fraud, participate in insurance support organizations, or
comply with lawful requests from regulatory and law enforcement authorities. These include, for
example: affiliated companies, claims adjusters, medical providers, and program managers.
You have the right to make a written request of us to correct, amend, or delete any recorded personal
information we have on you. We will, within 30 business days from the date we receive your request,
either correct, amend or delete the disputed portion of the information or we will notify you of our
refusal to do so, the reasons for refusal, and your right to file a supplementary statement disagreeing
with out refusal. If we refuse to correct, amend, or delete personal information, you have the
right to file a concise statement setting forth what you think is the correct, relevant, or fair
information and a concise statement of the reasons you disagree with our refusal to correct, amend,
or delete the information. If you file either statement, we will file the statement with the disputed
information. In this case, anyone reviewing the disputed information will also receive your statement.
We will also furnish your statement to any persons or insurance support organizations to whom we previously
provided the personal information in dispute.
If we agree to correct, amend, or delete the recorded personal information about you, we will notify
you in writing. We will furnish the correction, amendment or deletion to any person you specifically
designate who may have, within the preceding two years, received the information from us. The correction,
amendment, or deletion, will also be furnished to any insurance support organization which systematically
receives such information and still maintains it about you and to any insurance support organization that
furnished us with the information that has been corrected, amended, or deleted.
